John leaves behind cherished memories for friends, family, and all who had the privilege of knowing him. John’s life was marked by connections and experiences that touched those around him. Though he is no longer with us, the moments shared, the laughter, and the support he offered will continue to resonate. All who knew John will carry forward his memory in their hearts.
He was predeceased by: his wife Michelle A. "Mimi" Kozak (Gendron); his parents, Alfred Kozak and Lucille Kozak (Snow); and his siblings, Gerald Kozak, Walter Kozak and Marylou Skelhey.
He is survived by: his children, John M. Kozak II of Hopedale and Kim M. Cook (Jonathan) of Mansfield; and his grandsons, Joshua and Thomas. He is also survived by many nieces and nephews.
Visitation will be held on Saturday, October 24th 2026 from 10:00 AM to 11:00 AM at the Buma-Sargeant Funeral Home (42 Congress St, Milford, MA 01757).
