He is survived by: his significant other Katie Angers; his children, Layla Rain of Wales, Ella Sage of Wales, Dylan River of Wales, Gavin Forest of Wales and Nicholas Talon of Wales; his mother Linda Koslowski; his siblings, Travis Koslowski of Brimfield, Matt Koslowski of Southbridge and Jessica Sivula Koslowski of Holland; his parents-in-law, Richard and Marie Angers of Sturbridge; his nieces and nephews, Jack, Bella, Addylin, Gianna, Walker and Willow; his aunt Lynda Garieri of Sturbridge; Willam Koslowski of Wales. He is also survived by his wife and children.
