He was predeceased by: his parents, Lester A. Cook and Anna M. Ryder (Lyman); his son Christopher Cook; and his brothers, Stephen Cook and Ronald Cook.

He is survived by: his wife Sylvia Church (Church); his sons, Jon Cook (Dianne) and Jeffrey Cook (Sandie); and his grandsons, Jason Cook, Daniel Cook and Joseph Cook.
