She is survived by: her former husband Duane Kozak; her brother-in-law Alan Kozak; her sisters, Sharon Moomey (Rusty Moomey) and Maureen Regan (late Gerald Regan); her nieces, Lora Becker (Tom Becker) and Jennifer Hoffman (Jeffrey Hoffman); her nephew Steven Kozak (John Perell); and her grandchildren, Caroline, Katie, Kurt Becker, Addie and Alex Hoffman. She is also survived by several nieces and nephews.
In lieu of flowers, the family kindly requests donations to St. Jude Children's Research Hospital or The Luminis Health Gilchrist Lifecare Institute (in Memory of Carol Kozak).
