He was predeceased by: his parents, Stanley Dvorak and Eleanor Dvorak. He is survived by: his wife Evelyn Dvorak; his daughters, Janet Szarka (Jack) and Barbara Roginski (Tom); his grandsons, John Szarka, III (Pamela), Danny Szarka (Heather) and Steven Szarka (Danielle); his great grandchildren, Everett, Bentley, Olivia, Landon, Wilder and Owen; and his brother Phillip Dvorak (Marcia). He is also survived by many loving nieces and nephews.
