"Andrew" Booth (Amy Collins); her parents, Charles Cusick and Eula Cusick; her brother Jeffrey Cusick; her sister-in-law Charlene Augustine; her close friend Betty Beard (Steve); and her granddaughter Penelope. She is also survived by her nieces, nephew, great-nephew, and great-niece.
