He is survived by: his wife Terri; and his daughters, Lori Mellendick (Bill), Stephanie McClure (Scott), Niki Yurkow (John), Natalie Trott (Rick) and Amy Crouch (Jason). He is also survived by 11 grandchildren; and two great-grandchildren.
