He is survived by: his wife Linda Dvorak; his children, Michelle Dvorak of Chesapeake City, MD and John Anthony Dvorak (Rosella Hunter) of Fair Hill, MD; his siblings, Roger D. Dvorak of Elkton, MD, Thomas J. Dvorak of Hiawassee, GA, Michael C. Dvorak of Hiawassee, GA and Debra L. Cooper of Hiawassee, GA; and his son-in-law George Bostwick.
