He was predeceased by: his parents, Charles Polcak and Evelyn Mae Deering Polcak; his wife April Polcak; his sons, Brian Tom Polcak and Mark Charles Polcak; and his siblings, James Sladek and Patricia Richardson.
He is survived by: his daughter Deborah Lynn Showalter (Shawn); and his grandchildren, Tyler Polcak (Alexis), Jordan Polcak, Jacob Polcak and Ryan Polcak.
