He was predeceased by: his parents, Warren Dale Phillips, Sr. and Lenora Phillips (Williamson); and his sister Nancy Phillips. He is survived by: his children, Abby Justice, Benjamin Phillips (Ashlee) and Matthew Phillips (Megan); his brother John Phillips (Robin); his grandchildren, Samuel Justice (Erica), Eric Justice (Lydia), Michelle Phillips, Jake Rice, Kylee Rice, Hayven Rice, Ellis Phillips, Gage Rice, Benjamin Phillips and Tripp Phillips; his great grandchildren, Morgan and Rogue Justice; his nephew Pasha Phillips; and his niece Julia Phillips.

In lieu of flowers, the family requests memorial contributions be made to the funeral home to offset expenses.
