He was predeceased by: his parents, Jack Sr. and Cornelia (Connie); and his sister Beverly. He is survived by: his wife Rhonda; his children, Blair (Terra), Kaeden (Patrick Offenbacher) and Jack III; his grandchildren, Carly (Chase Clines), Marah and John; his great grandchild Delta Clines; and his sister Patricia Joyner (Larry). He is also survived by one niece and several nephews.
