He was predeceased by: his parents, Dewey Hawk and Nellie Hawk (Cosner); his former wife Rebecca; his son Carl DeWayne; and his siblings, Eugene Hawk, Ruth Janoski and Dorothy Hoeck.

He is survived by: his wife Irene Hawk (Cosner); his children, Gary Hawk (Jackie), Evelyn Hawk of Mount Storm and Diane Junkins (Denny) of Mount Storm; his grandchildren, Brian Hawk (Rebecca), Kevin Junkins (Emily), Randy Junkins, Caleb Hawk, Brianna Hawk, Rebecca Hawkinberry (Nick), Joseph Hawk, Benjamin Hawk, Christopher Sparbell, Lance Sparbell and Patrick Sparbell (Alycia); and his siblings, Loren, Larry, Jerry Hawk, Sylvia Fulayter and Sandy Waybright. He is also survived by several nieces and nephews.

Due to the ongoing Covid-19 virus, only family members will be received at the Locust Grove Church in Mount Storm on Sunday, June 7, 2020 from 3:00 pm to 6:00 pm.
