He was predeceased by: his sister Sandra Forman; his niece Lori Forman; and his parents, Ida Klein and Leon Klein. He is survived by: his nephew Brian Forman.
A funeral service will be held on Sunday, June 22nd 2025 at 10:00 AM at the Fero Memorial Gardens (5891 N Lecanto Hwy, Beverly Hills, FL 34465).
