He was predeceased by : his parents, John Frank Horak and Margaret M. Soellner Horak; his wife Jeanette "Jean" Corinne Horak; and his siblings, Martin G. Horak and Madeline Klein.
He is survived by : his daughters, Barbara Jean Parks (Michael), Lauri Ann Rosso (Tom), Juliana Marie Perez (Phil), Michele Louise Calhoun (James), Mary Elizabeth Swack (Phil) and Patricia Ellen Rose (Tony); and his sister Janice Mohorovic (Jake). He is also survived by 20 grandchildren, many great-grandchildren.
