She is survived by: her mother Lorraine Z Lavoie (LaPlante) of West Chapman; her husband James Kurtz of Eddington; her daughters, Nikkol Bull of Bangor and Angela Livermore (Craig) of Alexander; her grandchildren, Forrest Carter and Iain Livermore; her father Phillip Lavoie; her sister Nancy Allen (George) of West Chapman; her nephew Joshua; her son Maverick; and her pet Sasha.
In lieu of flowers, the family asks anyone attending the celebration of life to bring with them a bag of dog or cat food, or make a monetary donation to the Bangor Humane Society 693 Mt. Hope Ave Bangor, ME 04401 in Phyllis's name.
