He was predeceased by : his parents, Walter Spack and Evelyn Spack (Rogers); and his siblings, Steven, Zigmond, Suzy Spack, Mary Sadler and Nancy Perry.

He is survived by : his wife Evelyn Spack (Storer); his sons, James Spack, John Spack, Joseph Spack (Gail) and Forrest Spack Jr. (Angie); his brothers, Gary Spack and Terry Spack (Joanne); his grandchildren, Heather Twiggs (Don), Julie Spack (Ryan Buff), Justin Spack (Jessica), Kimberly Spack, Brittany Spack, Jason Spack, Stacy Spack, Corey Spack, Alicia Spack and Emily Spack; and his great grandchildren, Camryn, Tyler, Jaden, Jordan, Madison, Jailyn, Lilly, Elaina, Olivia, Caraline, Oaklynn, Jameson, Tia, Gavin, Liam and Griffen. He is also survived by many nieces and nephews.
