He was predeceased by: his parents, Nicholas "Pierre" Koutsivitis and Dorothy Koutsivitis (Schaivi). He is survived by: his sons, Anthony and Nickolas; his brother Charles Koutsivitis (Leslie Simonds); his pet Lucy Mae; and his former wife Cissy Forgione.
