He is survived by: his mother Helen Mary Mazaitis; his siblings, Eric Keith Adkins, Andrea Elizabeth Thomason and Kristen Leigh Kottke; his fiancée Gabriela Gomez Morales; his nieces and nephews, Alessa, Cade, Ashley Nina, David Jr. and Airen; his aunt Lisa; his cousins, Steven, Joey, Nichole and Becky; his pet Lilly; and his close friends, Alex Alakson and Demery Anderson.
Memorial contributions can be made in memory of David to a suicide prevention organization of the donor's choice.
