He was predeceased by: his father Charles Cornish. He is survived by: his mother Judy Cornish (Speer); his girlfriend Sherry Olmstead; his daughters, Ashley Cornish, Alyssa Yearty (Bryan), Aimee Cornish and Hannah Cornish; his sister Wendy Thompson (Mark); and his nephew Ryan. He is also survived by six grandchildren.

Chuck benefited from a heart transplant and the family therefore asks that in lieu of flowers, memorials be directed to the American Heart Association (http://www.heart.org) or the Lung & Heart Transplant Foundation (https://www.lungheart.org).
