He was predeceased by: his mother Mary Bartoszek; and his grandparents, Mary, Rene Beauvais, Frances and Edward Bartoszek.
He is survived by: his father Edward Bartoszek; and his sisters, Jennifer Madasz (Jason) and Jill Foxworthy. He is also survived by several nieces, nephews, aunts, uncles and cousins.
In lieu of flowers, the family asks that donations be made to the American Heart Association in Jeff's memory.
