He was predeceased by: his parents, Peter Kostus and Virginia Kostus (Herek); his sisters, Agnes and Rose; and his father-in-law Ray Stasik (Rita).
He is survived by: his wife Shirley Kay; his children, Brian (Anne), Matt (Lyn) and Ashley Kostus; his grandchildren, Cameron, Lauren, Kade Kostus, Mary, Audrey, Bennett Kostus, Jack, Drew and Cole Goik; his sisters, Cindy Kostus and Sue Jones (Jerry); and his siblings-in-law, Thomas Stasik (Nancy), Dick Stasik (Jan), Bette Pierson (Bob), Jim Stasik (Jackie) and Barb Gwizdala (Ron).
The family would like to ask that in lieu of sending flowers, you consider a donation to the Aleda E. Lutz VA Medical Center, Our Lady of Częstochowa Parish or simply call a loved one about something totally random that makes you think of them or crank up Chicago or Earth, Wind and Fire while doing something you enjoy - Tom would love it.
