He was predeceased by: his parents, Ted Kulik and Theresa Kulik; and his uncle Chester Blaszcik. He is survived by: his wife Sandi Kulik; his children, Jeff Kulik (Paula) and Jody Kulik (Kirsten); his grandchildren, Destinee, Jacob, Chase and Skye; and his great grandchildren, Aireeyona, Kannin and Kendall.
