She is survived by: her husband Greg; her children, James Kuzma (Marlo Ellsworth), Jessica Blythe (Ashlen) and Matthew Kuzma (Maria); her grandchildren, David, Kaylin Blythe, Aiela, Holden Kuzma, Brandon, Ariana and Thane Ellsworth; and her siblings, Harold Johnson (Karen) and Kathy Sands (Terrie). She is also survived by many extended family members and dear friends.
