He is survived by: his sons, Jonnie Jay Sam III (Rochelle) and Keith Allan Sam; his grandchildren, Gabriel Sam of East Lansing, MI, Kaylin Sam of Ann Arbor, MI, Jayston Arnold of Bear Lake, MI and Trista Arnold of Manistee; and his siblings, Jessica, Kris Steinberg of Brethren, MI, Janine Sam, Brian Bower of Byron Center, MI and Jeffrey Sam of Manistee, MI.. He is also survived by seven nieces and six nephews, and great nieces and nephews.
