He is survived by: his wife Jennifer; his children, Jason Cook, Edward Cook (Krista), Jesse Cook (Sara), Miriah DeKold and Scott DeKold; his grandchildren, Josie Cook, Carson Cook and Trent A. Patterson; and his brothers, John Cook and Jim Cook (Debbie). He is also survived by five children and three grandchildren.
