He is survived by: his mother Lucille Chittick (Eathorn); his wife Lori; his children, Daphne Hajek (Danny), Lindy Chittick and David Chittick, Jr.; his grandchildren, Daniel Hajek (Ashley) and Paige Hajek; his great grandchild Steven Hajek; and his siblings, Ronny Chittick (Katie), Joe Chittick, Tina Riley (Dave) and Cindy Harvey (Gary). He is also survived by many nieces and nephews.
