He was predeceased by: his parents, Carl A. Forman and Melva Jean Forman (Nickerson); and his granddaughter Clare Forman. He is survived by: his wife Lori Lynn Petrie; his children, Carl W. Forman (Jaclyn) and Miranda Toner (Joe LaLonde); his grandchildren, Hailey Forman, Chloe Forman, Payton Toner, Finnley Toner, Hudson Toner, Wyatt Toner and Charlie LaLonde; and his siblings, Lorie Pennell (Kirk), Tammy LeValley (Al), Kyle Forman (Jen) and Chad Forman (Bonnie). He is also survived by several nieces and nephews.
Visitation will be held on Thursday, October 9th 2025 from 3:00 PM to 7:00 PM and on Friday, October 10th 2025 at 10:00 AM at the Ransford Funeral Home & Cremation Services - Pigeon (7356 Michigan Ave, Pigeon, MI 48755). A funeral service will be held on Friday, October 10th 2025 at 11:00 AM at the same location. A burial will be held on Friday, October 10th 2025 at the Caseville Township Cemetery.
