He was predeceased by : his parents, John J. Sladek and Rose Sladek (Fratrick); his wife Beverly Cunningham; and his sister Rosemary Caposieno. He is survived by : his children, Barbara Mossner (Paul) of Manchester, IA, James Sladek (Rosalie) of Butte, MT, Gerald Sladek (Lisa) of Charlevoix and Joseph Sladek (Laural) of Charlevoix; and his siblings-in-law, Anthony Caposieno of Homosassa, FL and Sharon Woodbury (Janice Farmer) of Ironton.
