He is survived by: his wife Kathie; his sons, Matt (Jessica Hix) and Rob (Shannan); his granddaughters, Evelo and Annabelle; his siblings, Cathy Cook (Dean), Jim (Lisa) and Bill Cook (Meredith); and his siblings-in-law, Christi Dutcher (Tom), Mike Fulton (Janice) and Richard Butman. He is also survived by many nieces, nephews and cousins.
