He is survived by: his parents, Kristen Antkoviak (Howard) and Eddie Antkoviak; his sisters, Ella and Maci; his grandparents, Stanley of Cheboygan and Maureen Antkoviak of Cheboygan; his aunt Mary Bur (Scott) of Cheboygan; his uncle Kevin Howard (Cathy) of Willis; and his cousins, Ashley McMurry (Sam) and James Bur.

For those who wish to honor Jack's memory, the family requests contributions be made to the Cheboygan County Humane Society.
