He is survived by: his wife Judy Hazel; his children, Cheryl Moore (Greg) and Matthew Hazel (Shantel Ward); his grandchildren, Justine Johns (Jeremy), Tyler Moore, Matison Hazel and Jordan Moore; his great grandchildren, Jaelynn and Jackson; his brother Donald Hazel (Cathy); and his pet Bella.
