He was predeceased by: his parents, Clarence "Duane" Smith and Fern Smith (Potter). He is survived by: his wife Brenda Smith of Angola; his children, Chuck Smith (Tina) of Angola and Shelly Johnson (Steve) of Angola; his grandchildren, Michaella Ash (Conner), Hannah Johnson and Emily Weicht (Bryce); his great grandchild Brooks Ash; his brothers, Darrel Smith (Mary) and Dennis Smith (Nancy); and his stepsiblings, Linda Lindquist (Mike), Gerry Coplin (Lisa) and Bonnie Jones (Vern). He is also survived by many nieces and nephews.

Memorials may be directed to Parkview Hospice or the Steuben County Humane Society.
