He was predeceased by : his parents, Walter Koszyk and Margaret Koszyk (Geotz); and his brothers, Walter Koszyk, David Koszyk and Jackie Koszyk.

He is survived by : his wife Deborah Koszyk; his children, Brian Koszyk, Michael Koszyk Jr. (Barbara), Walter Koszyk (Dulce), Karen Fox (Josh) and Christina Ochota (Dave); his sisters, Susan Grere (Michael) and Judy Koszyk; and his grandchildren, Addison Fox, Alivia Fox, Casen Fox, Jessica Ochota and Joe Ochota.
