He is survived by: his parents, Michael Cook and Elizabeth Eremia; his wife Cindy; his children, Nicholas Cook and Dakota Cook; his stepchildren, Frankie Stinson and Courtney Stinson; his stepgrandchildren, Matthew Stinson, Jonathan Nizzola and Emmie Nizzola; his sister Kathy Cook; his aunt Kay; his uncles, Lee Ackerman, Jerry Eremia and John Eremia; his nieces, Jacci Keeley and Ricci Keeley; and his pets, Harley and Piper Lou. He is also survived by several nieces and nephews.
