He was predeceased by: his parents, Joseph J. Cook and Mabel H. Cook (Dwyer); his siblings, Louis Cook, Mary Boak, Julie Fedewa and Jerry; his sisters-in-law, Helen Randt and Marie Feldpausch; and his daughter-in-law Shantell Cook.
He is survived by: his wife Anne Cook (Fedewa); his children, Thomas, Laura, David (Jessica) and Dennis (Denise); his grandchildren, Caitlyn, Alex, Adam and Rachel; his sisters, Theresa Angwin and Karen Freauff; and his in-laws, Francis Fedewa, Ernie Fedewa, Margaret Coats, Bernadette Geahan and Kenneth Fedewa. He is also survived by nieces and nephews.
