He was predeceased by: his parents, Thomas Horak and Mary Horak; his parents-in-law, John and Anna Kusnier; his siblings, Mary Frances Kasparek and Jim Horak; and his brothers-in-law, Joe, John and Jerry Kusnier.
He is survived by: his children, Margaret White (Greg), Joe (Stephanie), Chris (Debbie), Paul (Vicki), Mark (Sue), Theresa Sopocy (Steve) and Monica Spicer (Mike); his wife Margaret Ann (Kusnier); his extended family, Helena, Frantisek Rozsypal and Kamil Rozsypal (Veronika); his sister Barbara Seybert (Terry); his siblings-in-law, Marsha Horak, Diane Kusnier, Mary Jane Holmes, Anna Anderson (Chris), Frank Kasparek, Robert Kusnier and Rick Kusnier (Carol); and his extended family Rev. Michael Kuchar. He is also survived by his seven children and their spouses, along with his grandchildren and great-grandchildren.
In lieu of flowers, donations may be directed to St. Cyril Catholic Church or the Bannister Knights of Columbus, continuing Jerry's lifelong commitment to supporting families in need.
