He is survived by: his parents, Laura Vizineau (Kositzky) and Scott "Butch" Vizineau; his siblings, Courtney Vizineau, Shae Vizineau (Zach Villeneuve) and Jaxon Vizineau; his siblings' children, Crew Villeneuve and Blakely Villeneuve; his aunts and uncles, Jody Mathias, Michelle Arnott (Brian), Kris Zubek (John), John Kositzky (Lisa), Joe Gonsowski (Laura), Mark Gonsowski (Michelle), Brian Rhoads and Tom Schorn (Brandy Bougie); his grandparents, Kim, Greg Schorn, Joan, Ross Rhoads, Neil Kositzky and Sheryl Bruce; and his great-grandparents, Robert and Gladys Schorn. He is also survived by cousins and numerous close and special friends.
In lieu of flowers memorial donations may be directed to the family to set up a memorial in Maddox's name.
