He is survived by: his wife Jennifer Miller; his sons, Jim (Tracey Miller) and Chris Miller; his grandchildren, Joshua, Ashley and Danielle; his great grandchild Mackenzie; and his sister Geneva Wagner.
In lieu of flowers, the family kindly requests that memorial donations be made in Jack's name to two charities that held a special place in his heart: the Gary Sinise Foundation (https://www.garysinisefoundation.org) and the American Heart Association.
