He was predeceased by: his grandparents, Fred, Pat Lucy, Clarence, Dolores Radcliff, Ralph and Florence Haxer; and his close friend Jason Oliphant.
He is survived by: his parents, Linda Radcliff and Joseph Haxer; his wife Stefanie Powell-Haxer; his children, Lia, Malina, Lilly and Jaxon; and his siblings, Laura (Janelle), Joshua (Shelby), Alesha (Theodore), Neal (Jeri), Steve (Lindsay), Paul (Lauren), Jaclyn (Jason), Nick and Joe. He is also survived by his family and friends.
Memorials are suggested to Stefanie to be placed in a trust for David's children.
