Jack and Amanda M. Jack; his sister Kathleen Jack; his siblings-in-law, David P. Foster (Becky) and Paula Carlson (Steve); and his close friends, James Gunter, Rich Renshaw, Kevin Burge, Tina Jacob and Lynne Stewart. He is also survived by other loving family members and good friends.
