He is survived by: his wife Helen Samples; his son Mark Samples (Monica); his grandchildren, Alexis and Blake Samples; his cousin Rick Lewis (Laura); and his siblings-in-law, Maryann Cross, Virginia Fetterman, John Ardelean (Bonnie) and Rosemary Clay (Harvey). He is also survived by nieces, nephews and other dear relatives.
