He is survived by: his parents, Daniel Berlin and Margaret Berlin (Scoble); his wife Jennifer Hunt; his sons, Dawson and Evan; his sister Lori (Vince); and his sibling's children, Elizabeth, Edward, Nathan and Jordan.

Contributions may be made to the American Diabetes Association and American Kidney Foundation.
