He was predeceased by: his parents, Gerald R. Balczak and Melissa Balczak; his siblings, Ted Balczak and Coleen Bent; and his mother-in-law Marilyn DeWolf.

He is survived by: his wife Donna; his children, Jerry Balczak and Jenna Spanding (Jon); his grandchildren, Chloe and Jerry Jr.; and his siblings, Tony Balczak (Shelley), Jack Bent, Chris Balczak (Tammi), Kevin Balczak (Patty), Ben Balczak (Sandy), Scott DeWolf (Lisa), Debbie Blok (Jim), Dottie Jacobs (Chris), Denise Rumsey (Carl), Tony DeWolf (Jen) and Mark DeWolf (Angie). He is also survived by many nieces, nephews and dear friends.
