He was predeceased by: his parents, Robert Chlebek and Margaret Jean Chlebek; his grandmother Mattie; and his brother Dick Chlebek (Julie). He is survived by: his daughter Pam Jenks (Chris); his grandchild Lauren; his daughter Amanda Blakeslee (Nick); his grandchildren, Brooklyn Vivoda, Jackson, Dr. Laura Welcher (Greg), Remington and Charley; and his pet Mocha. He is also survived by many nieces, nephews, and cousins.
