He is survived by: his husband John Folkertsma; his parents, Stanley Ciolk and Carol Ciolk; his siblings, Dan (Jaci), Adam and Stacy Potter (Matt); Jan Schichtel, Jaclyn Bromley and Jill Forton. He is also survived by numerous aunts, uncles and cousins; and John's family members.
