He is survived by: his parents, Josh Kooistra and Connie Kooistra of Ada; his brothers, Ben and Sam; his grandparents, Scott, Sue Kooistra and Beth Bahnsen; and his aunts and uncles, Jason (Heidi Kooistra), Jared (Betsy Kooistra), Jessica (Joel Petersen) and Amanda (Ross Weenum).
In lieu of flowers, contributions in his memory may be made to the Jason Kinzler Family First Foundation.
