He was predeceased by: his parents, Cyril Ptak and Frances Ptak (Novakoski). He is survived by: his brother David; and his friends, Dan, Lori Kowalewski, Kelsey, David Stockton, Sloane, Tom Milanowski and Sue Jankowski.
