He is survived by: his wife Susan Scarlett Middleton; his children, Heather (Mike Mathery), John (Julie Middleton Jr.), Peter (Cassidy Middleton) and Andrew (Tammy Middleton); and his siblings, Arthur, JoAnne Middleton 3rd, Alice, Don Westbrook and Katherine Booth. He is also survived by 11 grandchildren; 3 great-grandchildren.
In honor of John and in lieu of flowers, memorial contributions may be given to Wings of Hope Hospice, 530 Linn St. Allegan, MI 49010 or a charity of one's choice.
