He was predeceased by: his father Arlen; and his grandson Everett Matthew Hetherington. He is survived by: his wife Jo Ann Cook; his children, Stephen Jason Ashe (Renee), Aaron Lee Cook and Sarah Hetherington (Craig); his grandchildren, Greaydon Hetherington, Greyson Cook, Maycee Cook and Harrison Hetherington; and his siblings, Brenda Leaman (Bruce) and Gary Cook (Carol).

In lieu of flowers, memorial contributions can be made in his name to Crossroads Apostolic Church.
