He was predeceased by: his parents, Robert R. Addison and Marian L. Addison (Turk); and his brother Daniel R.. He is survived by: his spouse Shirley Ellen Cavender; his children, Dane Lawrence and Michelle "Shellie" Kathryn Cook; his granddaughter Bailey; his son-in-law Sean Cook; his grandson Dalton James Cook; his nephew Steven T. Addison (Diane); his great-nephew Stephen; his great-nieces, Ashley (Nobriga) and Alicia; his nieces, Danielle Addison, Kelly Russell, Tricia Prusakiewicz and Betsy Cavender; and his nephew Mike Cavender.
